RTI investigated catastrophic failures of a power station cooling tower which had recently undergone installation updates to their fan blade system.

Our experts determined the failure was caused when a lightning rod vibrated out of its sleeve and fell into the cooling cell below, resulting in systemic failures of adjacent cells and fan blade assemblies.

The vibrations causing the lightning rod to detach were a combination of an imbalance in the new fan blades and the deteriorated condition of significant portions of the cooling tower deck.
